Return Merchandise Authorization Form

This online form may be used to initiate a Return Merchandise Authorization (or RMA). An RMA is required to return any item to AVI-SPL.

Please complete the form below and click the submit button. Submit one RMA for each item you are returning. If you do not know the answer to one of the questions, please enter "unknown."

A customer service representative will contact you shortly. Once authorized, we will send you an RMA number and return instructions by email. Do not ship any products back to AVI-SPL without a valid RMA number. If you have problems submitting this form, please call 800-282-6733 ext. 2214.

Please note: All returns for credit are subject to a 15% restocking fee and are allowed only within 30 days from the date of purchase.

Whenever returning an item to AVI-SPL, we require the following packing and shipping methods:

  • Projector lamp hours cannot exceed 4 hours of usage.
  • Please return all products in their original packaging with original documentation.
  • Clearly place the RMA # on a label and attach it to the outside of the box.
  • Please do not write the RMA # directly on the box.
  • Include the RMA # in the "Ship To" address line.
  • Customer pays shipping costs and must insure the package for the purchased value. Acceptable shipping carriers are UPS, FedEx, DHL, or Airborne. Please do not send return merchandise via U.S. Mail or any other method; it will be refused. Products returned by U.S. Mail will not be credited or exchanged.
